Description of the Program:

  1. Cardholders will earn 1 “Point” for every $1.00 in “Net Purchases” made with the My5StarRewards Credit Card (“Card”) issued by 5Star Bank Points may be redeemed by the Cardholder for “Rewards”, consisting of travel, merchandise, gift cards and cash back. Points will be accumulated and posted once a week to an account maintained and managed by Consumer Benefit Services, Inc. (the “Administrator”)
  2. Accrual of Points may begin on the date the My5StarRewards Program begins or the date the Card and Card Agreement are first received by the Cardholder, whichever comes later.
  3. Point earnings are based on the new net retail purchase transaction volume (i.e., purchases less credits, returns and adjustments) charged to the Card during each periodic billing cycle ("Billing Cycle") by the Cardholder(s) ("Net Purchase(s)"). Net Purchases are rounded to the nearest dollar and are subject to verification. If a transaction is subject to a billing dispute, the Points value of the transaction will be deducted from the Points total during the dispute period. If the transaction is reinstated, Points will be reinstated.
  4. Points cannot be transferred from one person’s account to another person (including upon death or as part of a domestic relations matter). Points from multiple consumer accounts cannot be added together. Companies have the authority to allow individual billing or corporate billing for the purpose of this rewards program. Validly earned and unexpired Points posted to a lost/stolen account will be moved to the Cardholder’s new account.
  5. Points may not be combined with any other loyalty/frequency reward program.
  6. Points will not be earned or accumulated for cash advances, convenience checks, balance transfers, traveler’s check purchases, finance charges, late fees, annual fees, over-limit fees or transaction fees. 5Star bank has the option to include additional exclusions in the Rules governing the My5StarRewards Program.
  7. 5Star Bank reserves the right to award bonus Points to selected Cardholders.
  8. Points are not the property of the Cardholder, and cannot be bought or sold in any way.
  9. Points are tracked and redeemable on a first-in, first-out basis. Points will expire five (5) years from the date of issuance.
  10. To redeem Points, Cardholder’s Card account(s) must be open (meaning not voluntarily closed, canceled or terminated for any reason); current (meaning there are no past-due balances on the Cardholder’s Card account(s) at the time of redemption request); Cardholder’s outstanding balance must not be over the credit limit; account must not have a revoked, charged-off or bankruptcy status; and the Card cannot have any other status preventing authorizations.
  11. The Cardholder agrees to release 5Star Bank, the Administrator, and its vendors from all liability for any injury, accident, loss, claim, expense or damages sustained by the Cardholder, associated with a Reward or use of Rewards while participating in this program and in the case of a Travel Reward, anyone traveling with or without the Cardholder, in connection with the receipt, ownership, or use of any Reward. The foregoing entities shall not be liable for consequential damages, and the sole extent of liability, if at all, shall not exceed the actual value of the Reward.
  12. The Cardholder is responsible for determining any tax liability arising from participation in the Program. Consult a tax advisor concerning tax consequences.
  13. 5Star Bank and the Administrator shall have no liability for disagreements between Cardholders regarding Points. Discrepancies about Point earnings are not treated as credit card billing disputes. 5Star Bank’s decision regarding Point discrepancies shall be final. Redeemed Points are deducted from the Cardholder’s Point balance as of the request date of a Reward.
  14. Points are maintained in an account managed by the Administrator. Points can be redeemed once the Cardholder earns sufficient Points for the Rewards offered under the Program.
  15. Points cannot be used to offset the Cardholder’s obligation to 5Star Bank.
  16. Points must be redeemed by the Cardholder, but may be used for another person. If a reward is to be shipped to an alternate address, the Cardholder must contact customer service at 866-903-9594 to complete the order.
  17. 5Star Bank reserves the right to disqualify any Cardholder from participation in the program and invalidate all Points for abuse, fraud, or any violation of the Program terms and conditions. 5Star Bank may make such a determination in its sole discretion.
  18. The My5StarRewards Program is void where prohibited by federal, state, or local law.
  19. 5Star Bank and the Administrator are not responsible for typographical errors and/or omissions in any Program document.
  20. 5Star Bank and the Administrator reserve the right to change the Terms and Conditions of the My5StarRewards Program. 5Star Bank has the option to restrict, limit, expire, or cancel redemption of Points at any time without prior notice.
  21. The My5StarRewards Program is a service provided by 5Star Bank and the Administrator. In the event of fraud, abuse of Program privileges or violation of the Program Rules (including any attempt to sell, exchange or transfer Points or the instrument exchangeable for Points), 5Star Bank reserves the right to cancel Cardholder’s membership in the My5StarRewards Program.
  22. My5StarRewards is a trademark of 5Star Bank.

Travel Rewards:

The Administrator’s travel redemption center is a full service agency and can assist with all travel needs including air travel rewards, hotel, auto, vacation and cruise reservations. They can be reached toll-free at 866-903-9594.

  1. All Travel must be redeemed through Administrator’s fully licensed redemption reservation center. Cardholders must have a valid credit card at the time of redemption.
  2. All airline tickets issued in exchange for Points are non-refundable and non-changeable after ticket issuance, without paying the standard fees charged by each airline. Changes are subject to authorization by the airline and subject to add-collects and fees charged by the airline and redemption center.
  3. Lost, stolen or otherwise destroyed airline tickets will not be replaced, without the Cardholder paying the standard fees charged by each airline.
  4. Cardholders may make additional travel reservations with the Administrator’s travel agency using their Card. The travel agency’s normal and customary fees will be billed to the Card for any additional services required by the Cardholder.
  5. All travel documents will be issued the same day the reservation is made.
  6. Airfares are not guaranteed if not ticketed immediately. All reservations will receive a fax or email the same day the ticket is issued. The Cardholder must report any corrections or discrepancies to the Administrator the same day that a reservation is made. This must occur by 9:00pm CST Monday through Friday, by 5:00pm CST on Saturday, and 2:00pm CST on Sunday. Any changes or corrections done the following day or thereafter are subject to all airline airfare charges, exchange fees and processing fees.
  7. Paper airline tickets are subject to the individual airline paper ticket fees.
  8. If a paper ticket is issued, the Cardholder has two options for delivery. The first option is to sign a waiver stating that the Cardholder accepts responsibility for a lost ticket, and then the ticket will be sent via US Mail. The second option is to pay a $15 shipping fee for the ticket to be sent via DHL overnight delivery. For Priority, Saturday and deliveries outside the 48 contiguous states, additional shipping charges will be assessed.
  9. The Cardholder is responsible for payment of any baggage charges, departure taxes or other charges that may have been assessed by governmental entities as a result of travel under the My5StarRewards Program.
  10. Administrator’s normal and customary fees associated with processing travel related services are billed to the Cardholder’s Card.
  11. 5Star Bank and the Administrator are not responsible for the performance by the airlines of the ticketed transportation. All reservations are made subject to the conditions of carriage, supply or business of the party providing the service, which include exclusions and limitations of liability. The airline industry is in constant flux and changes brought down by this industry are done quickly and without notice, therefore, Award redemption rules for air travel are subject to change without notice.
  12. Specific restrictions may apply to certain Travel Rewards.

DISCOUNTED AIRFARES. Cardholders may redeem 10,000 accumulated, un-expired Points for a $100 discount off of any scheduled major International or Domestic airline. The total ticket cost will be billed to the Cardholder’s Card with a $100.00 credit that shall be applied within 1 to 2 statement cycles.

AIRLINE TICKET. Cardholders may redeem accumulated, un-expired Points for a single, round-trip, lowest published airfare as follows:

  1. Each AIRLINE TICKET must be ordered through Administrator for one round trip coach class airline ticket on a scheduled U.S. or International carrier.
  2. All AIRLINE TICKETS must be for round-trip travel on the same airlines or Code Share airline.
  3. En-route stopovers are not permitted unless they are to make direct connections.
  4. Reservations for tickets also exclude the usage of charters.
  5. Reservations and ticketing must be made at least twenty-one (21) days prior to actual departure date.
  6. Actual travel may occur any time within three hundred and thirty (330) days after the reservation conditions in this Agreement are met.
  7. For domestic travel a Saturday night stay must be included in the travel itinerary.
  8. Reservations shall also be subject to airline seat availability on travel dates specified by the traveler.
  9. Administrator reserves the right to choose a major airline of their choice on which to reserve and ticket Cardholders for AIRLINE TICKETS.
  10. The travel agency has the right to book Cardholder’s reservation within 2 hours of Cardholder’s requested travel times for departures and returns.
  11. Each AIRLINE TICKET may not exceed the Ticket Cap, however Cardholder may move up to the next Point level as listed on the Airline Ticket Matrix and use that higher amount of Points.
  12. For ticket destinations and departures not listed on the AirlineTicket Matrix, the number of Points that will be required will be determined by matching the amount of the ticket with the appropriate Ticket Cap, while not exceeding it.

Merchandise and Gift Card Rewards:

  1. When necessary, the Administrator may substitute a merchandise Reward with an updated model of equal or greater value without advance notice. Cardholders will be notified of any change when ordering. The Administrator may remove certain items and may replace or remove certain sections within any My5StarRewards Program literature or website. All awards are subject to availability.
  2. The standard shipping time is 2-4 weeks for gift card orders and 4-6 weeks for merchandise orders. Some items may be received sooner. Multiple rewards may arrive at different times from different vendors.
  3. No shipments of merchandise can be made to APO/FPO or PO Box addresses, as most are delivered by UPS.
  4. Gift Cards are mailed via US Mail and therefore can be shipped to APO/FPO or PO Box addresses.
  5. Merchandise shippable by UPS will be available to all US territories. Items being shipped to Alaska, Hawaii, Puerto Rico, Guam, the US Virgin Islands or any foreign approved destination will have an additional freight charge billed to the Cardholder’s Card.
  6. Merchandise pictured in any My5StarRewards Program catalog or website may not necessarily reflect exact colors or models of actual awards due to printing variations and/or manufacturers’ updates. Information is accurate to the very best of our knowledge. 5Star Bank and the Administrator are not responsible for errors or omissions.
  7. Points required for award items are subject to change.
  8. Cardholders may exchange merchandise only in the event of merchandise defects or damage in shipment. Any exceptions, damages, or shortages must be noted on the delivery receipt before Cardholders sign to accept shipment of merchandise.
  9. All merchandise is covered by manufacturer’s warranties. Any such defect should be handled through the standard manufacturer repair facility as noted with product.

Gift Certificates:

  1. You may redeem Points earned in the Program for gift certificates. Mechanical reproductions, copies or facsimiles of gift certificates/cards will not be accepted. Additional terms and conditions appearing on the gift certificate/card are to be interpreted in accordance with, and are incorporated into, these Program Terms and Conditions.
  2. Gift certificate/card rewards will be mailed only to your billing address as reflected in our records. Gift certificates/cards are transferable and may be redeemed by any person.
  3. Gift certificates/cards must be presented to participating merchant partner for use. Gift certificates/cards are not exchangeable, refundable, or redeemable for cash and cannot be replaced if lost or stolen.

Cash Back Terms and Conditions:

The My5StarRewards Program provides a $100 cash back option, which can be redeemed with 12,500 Points or a $50 cash back option for 6,500 points. The cash back award is provided in the form of a check, which will be mailed within 4 to 6 weeks after the order is placed.
