5Star Group Level Term Eligibility Details

• US Citizens between the ages of 1 and 64 including your spouse and children are eligible to apply.
• Your dependent children are eligible for coverage through a separate certificate (ages 1 - 17) or through a Rider (under age 21, 23 if a full-time student).
