and contractors for the above. Plans are available for the whole
family with affordable contribution amounts.

Medical evaluation by an authorized paramedic or
medical doctor.
Within specific age groups and coverage amounts, detailed
results can be used from a complete medical history
and physical examination. |
Designed for members under age 45, coverage is on
a tiered contribution schedule - contribution amounts
increase concurrently with the age group increments.
Coverage ranges from $50,000 to $400,000 in units of
$50,000 and continues through age 70. |

contributions for the group plan are locked-in for
a specified amount of time.
The individual plan is available in 10, 15, 20 or 30-year
coverage. |
