We have simplified our death claim reporting process. Please note that if you respond online, you must complete the process in one sitting, as there is no ability to save your progress. If you choose to file online, you will receive an email confirmation upon submission. We will notify you if further information is required. Please be aware this notification does not guarantee payment of a death claim.

Before you begin

Please take a few moments to gather the following information. This will make the process easier for you.

 Deceased certificate holder information Beneficiary or next of kin contact information
  • Certificate number(s)
  • Date of death
  • Cause and manner of death
  • Social Security number
  • Name and address
  • Phone number
  • Email address
  • Date of birth and Social Security number of surviving spouse if applicable

  • If you have difficulty completing the form
  • Are a resident of the State of New York
  • Are filing an AFBA Child Rider claim
  • If the death claim has an Assignment*
  • Participate in the Worksite Employee Benefits Program
  • Participate in the National Guard State Sponsored Life Insurance (SSLI) Program

*  If the death claim  has an “Assignment,” please do not complete the online form. The Assignment will not be accepted. You must contact us. Failure to report an Assignment via telephone in a timely manner may cause death proceeds to be disbursed to the beneficiary of record. (this sounds HARSH)
