Please note: National Guard members, information received will be forwarded to the member’s National Guard Association or the appropriate service center for further processing.


Information needed to file

The following documentation is needed whether the claim is filed online or by phone. Upon receipt you will be contacted for information as needed.


  • Certificate number(s)
  • Date of death
  • Cause/ manner of death
  •  Social Security Number

Beneficiary or next of kin:

  • Name and address
  • Phone number
  • Email address
  • Date of birth and Social Security Number
